NOTE: Students who are already registered with OA do not need to complete the application to request Letters of Accommodation, to schedule meetings with OA staff, or to contact us with questions.


This application is for new students who would like to receive services and accommodations. Please complete the application in full - OA Accommodations Request Form.  Any information submitted is confidential and used for the purposes of approving reasonable accommodations.

After submitting your application, you will be taken to a second page to upload your disability documentation. Your application confirmation e-mail will also include a link for submitting documentation at a later time, if needed. Important: Your application is not complete and cannot be processed until you have uploaded your disability documentation to your application.
