Welcome to the Office of Accessibility (OA) New Student Accommodation Request Application. This application is for new students who would like to receive services and accommodations.

Students who are registered with OA do not need to complete the application to request Letters of Accommodation, to schedule meetings with OA staff, or to contact us with questions.

Please complete the application in full. Please note that any information submitted is confidential and used for the purposes of approving reasonable accommodations.

After submitting your application, you will be taken to a second page to upload your disability documentation. (Your application confirmation email will also include a link for submitting documentation at a later time, if needed.)

Your application is not complete and cannot be processed until you have uploaded your disability documentation to your application. If you have any difficulties or questions related to submitting your application or uploading files, please contact the Office of Accessibility at
