Students may find themselves missing classes due to different circumstances. The Absence Notification Form is designed to serve as a resource and central point of contact for students who need to notified instructors of their absence of more than one day, and one of the three causes listed in the Class Attendance Policy* for make-up opportunities. This is a courtesy that we provide to help in communicating a student’s absence, not an excused absence. The purpose of the Absence Notification Form is to verify documentation and report a student absence. Students can find the form on their Wingspan under Student Tools - Absence Notification.
* NOTE: Do not fill out this form if only one day is verifiable. Please speak with your instructor directly.
All absence e-mails will go directly to the student and the student will be responsible for forwarding that information to their instructors or WU employer. For Students Diagnosed with COVID - at home test results are not accepted. You must provide a medical provider’s note stating dates in which they request you be out of class.
