This form is to be completed by Greek organizations when a scheduled event is to take place on or off chapter premises and submitted by 5:00 p.m. two days before the event. For any event on Monday or Tuesday, an Event Notification form must be completed by 5:00 p.m. the previous Friday.

All fields must be filled out for event to be approved.
