Instructions for filling out the above forms

To open the online forms above, you need to have Microsoft Word installed on your computer. 

When you click on the link above to download the file, you may either print it out and fill in the blanks with a typewriter or by hand, or you can use Microsoft Word to do so. 

•      To print a blank copy of the form, go to the File menu and choose Print. 

•      To use Microsoft Word to fill in the form, go to the File menu and choose Save As. In the Save in: drop-down list, choose the location where you would like to save the file. To make the file easy to find later on, it is a good idea to choose Desktop from the list. Once you have chosen the location to save the file, click Save. Next, close your browser window and open the file with Microsoft Word. To enter information, click on the gray fields and then type your answers. You may also use the Tab key to move between fields. You can now edit, save, close, reopen and print this file just as you would any Word document.
