Updating Your Organization's Records

  • Advisor Agreement
    Fill out the PDF form, print, and sign. Deliver to Student Activities in 269 DiGiorgio Campus Center.
  • Officer Update Form
    (Note: This is NOT the annual registration form.) Only fill out this form and submit if you are updating your organization's officers because they have changed since you completed the Annual Registration form.
  • Roster Form
    Print out the PDF form. Have students fill out the form and sign. Deliver to Student Activities in 269 DiGiorgio Campus Center.

Setting up a Bank Account

  • Federal Identification Number
    Fill out this online form and submit to IRS to receive a federal EIN number, which is necessary to set up a bank account for your organization.
