Club Sports Handbook (PDF)

Accident Report (PDF)
To be completed as needed. This form should be filled out to record any injury sustained during club sport activities.

Allocation Request (PDF)
Form must be filled out to request allocations for the year.

Application and Registration Form (PDF)
This must be filled out completely with member names and contact information to be recognized as a club sport by Recreational Services.

Coach Agreement Form (PDF)
Each club most have a coach. This individual must sign this form agreeing to be the coach and the club president must also sign before returning it to Recreational Services.

Driver Contract Agreement (PDF)
Each club president must designate drivers for their club while traveling.  These drivers are the only ones allowed to drive to and from competition and events.  All drivers must complete this form before any travel will be approved.

Event Request Form (PDF)
In order to hold events or competitions at Winthrop, this form must be filled out two weeks in advance.

Inventory Report (PDF)
This report of all club equipment must be completed at the beginning and the end of each calendar year in conjunction with the office of Recreational Services.

Liability Form (PDF)
Each club member must complete and sign this form before they are eligible to participate in club sport activities. Club president must also sign this form for each participant and it must be returned to Recreational Services.

Practice Request Form (PDF)
Form must be filled out completely in order to reserve practice space at Winthrop.

Sample Constitution (PDF)
This is a guide to writing the constitution for a new club. Update and change this sample to meet the needs of your organization.

Travel Authorization Form (PDF)
Must be completed with each traveling club member's emergency contact information and returned to Recreational Services one week before the event.

Year End Report (PDF)
At the end of each school year, club presidents must fill out this form with detailed information as to their activities during the year.

Year End Survey (PDF)
Each club member is given the opportunity to voice their opinions regarding the status of the club for the year. These must be filled out at the end of each school year and returned to Recreational Services.
