- Low Income Form for Dependent Students
Completion of this form may be required by the Office of Financial Aid if income reported on the FAFSA by your parents appears insufficient to provide for basic needs. If you and your parents need to provide the information requested on this form, the financial aid office will notify you.

Complete this form to apply for the William D. Ford Federal Direct Parent PLUS loan program.
- State Grant-Scholarship Affidavit
For students who have Palmetto Fellows, LIFE, or SC Grant, they must sign an affidavit.
- Maintaining Satisfactory Academic Progress
This information provides guidelines for satisfactory academic progress required to maintain eligibility for student financial aid.
- Appeal Request for Satisfactory Academic Progress to Receive Aid
Complete this form to appeal the denial of your aid due to lack of satisfactory academic progress. If you completed the form mailed to you by our office, you do not need to complete this again. Remember to include the required documentation.
- Selective Service Registration Compliance Information
Most males age 18 and over are required to be registered with Selective Service. Your FAFSA data was compared to the Selective Service database and did not match. This compliance information is provided to assist male students in meeting the requirement to be registered for Selective Service.
- Statement of Registration Status with Selective Service
If you meet the criteria for exemption from selective service registration, complete either Part A or Part B on the form and return it to the Office of Financial Aid. If you are female, you have received this notice because you did not answer “no” to the FAFSA question, “Are you male?” Complete Part A of the Statement, sign and return it to the Office of Financial Aid; 119 Tillman Hall; Rock Hill, SC 29733.
- Verification of Veterans' Educational Benefits
If you reported that you will be receiving Veterans’ Educational benefits on your FAFSA, complete this form so that the Office of Financial Aid may verify the amount you will be receiving from July 1 through June 30.
