Programs of Study
A Program of Study (POS) serves as a degree checklist for graduation. Every student must have a POS on file with Records and Registration before graduation. Blank Program of Study forms are available below. Consult your advisor early in your graduate career to create your POS. From time to time you may need to change a POS. This is done by amending your POS with a "Request for Change in Program of Study" (pdf- 27 KB) form. All forms must be reviewed and signed by your advisor, the department chair, the Director of Graduate Studies, and a representative from Records and Registration.
All forms are available in pdf format.
